COVID-19 Health Screening Questionnaire

Effective June 1, 2021

Daily online health screenings are no longer required; however, individuals should perform a self-health check prior to coming to campus and stay home if sick.

We will continue to follow guidance from the CDC and our state and local health departments,  so it's vital that you check your Brazosport College email for updates before heading to campus.

COVID-19 CONTACT/EXPOSURE REPORT FORM

For individuals needing to self-identify as affected by COVID-19 or notify Brazosport College of COVID-19 incidents involving students, faculty, or staff, please visit www.brazosport.edu/coronavirus/report. 

Please note, the COVID-19 Contact/Exposure Report Form is separate and apart from the Health Screening Questionnaire. The COVID-19 Contact/Exposure Form should be completed if you have been exposed to someone or if you have tested positive for COVID-19.
